Trials PRISM and PRISM-PLUS ; demonstrated that the effect of AGGRASTAT alone on mortality at 7 and 30 days ; was comparable to that of heparin alone. RESTORE Randomized Efficacy Study of Tirofiban for Outcomes and Restenosis ; The RESTORE study n 2141 ; was a randomized, controlled comparison of AGGRASTAT and placebo, each added to heparin, in patients undergoing PTCA or atherectomy within 72 hours of presentation with unstable angina or acute myocardial infarction. The mean age of the population was 59 years; 27% were female. Two-thirds of patients underwent angioplasty for unstable angina and the remainder in association with acute myocardial infarction. Exclusions included anatomy not amenable to angioplasty, contraindications to anticoagulation see CONTRAINDICATIONS ; , platelet count 150, 000 mm3, and creatinine 2.0 mg dL. AGGRASTAT with heparin ; was initiated immediately prior to the angioplasty atherectomy at a dose of 10 mcg kg bolus over 3 minutes ; followed by an infusion of 0.15 mcg kg min along with a heparin bolus bolus of 10, 000 U, or 150 U kg for patients 70 kg ; . The infusion dose of AGGRASTAT is 50% higher than the dose used in the PRISM-PLUS trial. AGGRASTAT was administered for a total of 36 hours. In general, heparin was to be discontinued at the conclusion of the angioplasty atherectomy. Dr. Hoyt: Nearly all of these drugs have been tried on superior oblique myokymia. It is difficult to evaluate in superior oblique myokymia, because the disorder has a tendency to wax and wane on its own, independent of pharmacologic therapy. The only one that I've ever been impressed with that does much in superior oblique myokymia is gabapentin. The other ones don't seem do much of anything to it. There was a lot of excitement about scopolamine as a potential drug for nystagmus. There were transdermal scopolamine patches for seasickness, so it looked it like there was going to be a readily available therapy. What happened to it? Dr. Zee: Two things: the side effects are terrible. If you take those patches for more than a few days, you quickly become addicted to them, and getting someone off patches is a real problem. Arboviral infections constitute a large group of diseases caused by viruses currently about 80 known in humans ; defined by ecologic, epidemiologic, and clinical, rather than taxonomic, characteristics. The term "arbovirus" is a contraction of "arthropod-borne virus. " Strictly speaking, arboviruses replicate in and are transmitted by arthropods predominantly mosquitoes, but also ticks, sandflies, midges, and gnats ; a generalized arbovirus life cycle is shown in fig. 4-15 ; . However, there are some arbovirus-like diseases whose vector is still unidentified e.g., those caused by the Arenaviridae family ; and some whose early epidemiologic profile incorrectly suggested arthropod transmission e.g., Argentinean and Bolivian hemorrhagic fevers ; . These exceptions are noted, but for lack of a better characterization system, they are discussed here.
